Terms & Conditions
These terms are subject to reasonable rules and regulations, including, but not limited to available equipment, and hours/days of operation, which may be changed from time to time.
The practice reserves the right to change program policies from time to time as deemed necessary without written notice.
The initial membership term shall be a minimum of 12 months and is not refundable. Upon completion of the 12-month term, you may cancel 30 days prior to your next EFT due date by filling out a Membership Cancellation Form and processed in person.
The total installment dues are payable in 12 consecutive monthly installments of $89 beginning on the 1st day of each month and continue on a month-to-month basis.
Terms of Membership & Renewal Provisions
Following the initial 12-month membership term, for your convenience, this contract will be extended on a month-to-month basis with automatic EFT credit/debit card payments. Contract cancellation requires 30 days’ written notice prior to your next automatic EFT without incurring additional liability of one (1) month’s due.
Membership Cancellation Form must be processed in person. Member must not be in default and must be current on fees for cancellation to become effective.

Membership Details
The membership price of $39/month & $99 Enrollment Fee includes:

Exclusive invitation to members-only events.
Terms & Conditions
Membership is on a month-to-month basis and will auto-renew every month.
All membership fees are non-refundable.
Monthly membership fees are payable by credit card only. Members must supply Davam Aesthetics with a credit card to place on file for automatic monthly payments. Monthly renewal will be charged to the credit card on file.
Should a credit card payment be declined, member will be notified and asked to supply an alternate credit card for payment within 10 days. All benefits will be terminated until the account is brought back in to good standing. After the account is delinquent for more than 30 days, members will lose their membership.
Complete payment for services rendered must be made at the time of treatment.
Membership discounts cannot be combined with other offers or discounts.
Membership may not be transferred to any other person or entity at any time for any reason. The benefits of Membership will inure to the Member only during the active term of Membership.
Davam Aesthetics has the right to change these terms and conditions at any time for any reason at its discretion, including services and discounts offered.
Terms will be provided to each Member within thirty (30) days of any changes made. Membership will continue in full force and effect regardless of any changes to the terms and conditions contained herein.
Davam Aesthetics reserves the right to change, add, or remove services and/or products available at its facilities and does not guarantee any particular service, treatment, or product will be available to Members or other clients.
Davam Aesthetics reserves the right to review memberships and deny, suspend, or cancel your membership at any time if the terms of this membership agreement are not met.
As part of your Davam Aesthetics Tox Elite Membership you receive (1) complimentary facial for your birthday which can be redeemed within any day of your birth month.
Non –Used treatment cannot be rolled over to the following month.
Cancellation/Termination or Suspension of Membership
Davam Aesthetics reserves the right at any time to cancel or suspend the membership of any member in the event of the following:
The member commits a serious breach of the Agreement and/or Davam Aesthetics Rules and Policies.
Where any monies are due to Davam Aesthetics by the member remain unpaid for 30 days after its due date for payment.
The member knowingly provides false details when applying for membership and the false declaration would have reasonably affected Davam Aesthetics decision to grant the membership.
If Davam Aesthetics terminates the membership for any of the above reasons, they reserve the right to retain any monies received to cover any reasonable costs they have incurred as a result.
Our Membership agreement is a month-to-month agreement and requires a 30 day notice to terminate. As long as all dues are current, this notice will be accepted as your 30 day notice to terminate. Please email spa@davam.com to cancel membership.
